We recognise that no two clients are the same. Each person we support has a different set of complex healthcare needs.
When we put together a new client care package, we take time to meet the individual, their family and the existing healthcare team, to find out how we can best support them.
Often this will involve a transition from hospital, a rehabilitation unit or from another care provider.
We put in place a dedicated team of nurses and carers who have the necessary skills to meet the physical, mental, emotional and social needs of our client.
Our emphasis is on doing the groundwork ahead of the transition, including case management support where required, to ensure a smooth transfer back into the community – with a complete care package in place and ready to go from the moment our client arrives home.
We also recognise that nursing is only one aspect of the care our clients need. Our carers see the whole person, rather than just an illness or disability. We hand-pick our team, based not only on their nursing qualifications but also on their ability to empathise and make a connection with our clients.
We know that many of our clients – and their families – welcome the companionship and emotional support our teams provide. We actively support our clients to take an interest in hobbies or other pastimes and help them to live as full and independent a life as they are able to.
